I think the distinction, if there is any, is meaningless. Al-Shaiir violently hit a sliding QB which, even assuming he didn't hit his head directly, at a minimum caused whiplashing of Lawrence's head into the ground (and very likely a concussion). Saying he didn't hit his head is pretty meaningless in this context, which begs the question of why someone is bringing it up at all...
