1--Quarterback Situation
2--Barkley Situation
3--Daboll taking over play-calling
4--No Depth at key positions
5--Defensive Coordinator

Most of us already agree with most of these. For sure, not the way to build a roster.
I HATE TO INCLUDE THE WORD NASTY< BUT THAT IS PART OF BEING A WINNING FOOTBALL TEAM.

The biggest problem was the poor drafts.  If a GM does not draft well, there are limited alternatives available.

For example-  Schoen let Barkley walk and used the money he would have been paid to sign veteran O-linemen.   Had Schoen drafted better O-linemen, they could have used the money to retain Barkley instead of signing those veteran O-linemen.

The lack of quality drafts also struck to number 4 on Pat's list
